criticalSt. John's Wort raises brain serotonin, norepinephrine, and dopamine through reuptake inhibition and shows weak monoamine oxidase inhibition. Layered on a prescription MAOI, which blocks the breakdown of those same monoamines, the combination can push monoamine signaling to dangerous levels and is contraindicated because of the risk of serotonin syndrome and hypertensive crisis.
salt substitute + spironolactone
criticalMost salt substitutes replace ordinary table salt with potassium chloride, so they act as concentrated potassium supplements. Spironolactone is a potassium-sparing diuretic that reduces how much potassium the kidneys excrete. Using the two together can drive potassium high enough to cause dangerous, sometimes life-threatening heart rhythm problems (hyperkalemia).
maoi + tyramine foods
criticalMonoamine oxidase inhibitors block MAO-A in the gut and liver, the enzyme that normally breaks down dietary tyramine. Unmetabolized tyramine triggers a surge of stored norepinephrine, which can produce a hypertensive crisis (the 'cheese reaction') with severe blood pressure spikes, headache, and in serious cases stroke or death.
licorice tea + digoxin
criticalLicorice (Glycyrrhiza glabra) contains glycyrrhizin, which inhibits the renal enzyme 11-beta-hydroxysteroid dehydrogenase type 2 and produces mineralocorticoid-like potassium loss. The resulting hypokalemia increases the heart's sensitivity to digoxin, raising the risk of digoxin toxicity and dangerous arrhythmias.
oral contraceptives + st. john's wort
criticalSt. John's Wort induces CYP3A4 and P-glycoprotein, increasing the clearance of contraceptive hormones and reducing the effectiveness of hormonal contraceptives.
caffeine + ephedra

azathioprine + allopurinol

Taking the two together markedly lowers cyclosporine blood levels, which can render the drug subtherapeutic. This has caused documented acute organ rejection in transplant recipients, making the combination a contraindication.

The combination is generally well tolerated in healthy adults, with the main cautions involving MAO inhibitors, levodopa, and thyroid medication.
rhodiola + ashwagandha
synergyRhodiola rosea and ashwagandha are both adaptogens that act through different mechanisms. Rhodiola tends to be energizing and anti-fatigue, working on monoamines and the HPA axis, while ashwagandha tends to be calming and helps normalize cortisol. Many people pair them so that rhodiola covers the activating, daytime side of the stress response and ashwagandha covers the calming, evening side. smoking + varenicline

saffron + curcumin
synergySaffron (Crocus sativus) and curcumin (from turmeric) both have antidepressant effects through partly complementary mechanisms: saffron modulates serotonin and dopamine reuptake and increases BDNF, while curcumin reduces neuroinflammation, supports monoamine balance, and normalizes the HPA axis. A randomized, double-blind, placebo-controlled trial found a saffron plus curcumin combination significantly improved depressive symptoms versus placebo in adults with major depression.
