cyclosporine + st. john's wort
criticalSt. John's wort is a potent inducer of CYP3A4 and P-glycoprotein, the enzyme and transporter that clear cyclosporine. Taking the two together markedly lowers cyclosporine blood levels, which can render the drug subtherapeutic. This has caused documented acute organ rejection in transplant recipients, making the combination a contraindication.
alcohol + tramadol
criticalTramadol combined with alcohol produces additive central nervous system and respiratory depression, and the combination lowers the seizure threshold, increasing the risk of convulsions, serotonin-related reactions, and life-threatening overdose. Tramadol's serotonergic and noradrenergic activity makes this pairing more hazardous than alcohol with a typical opioid.
alcohol + diazepam
criticalDiazepam (Valium) and alcohol are both central nervous system depressants that act on the GABA-A receptor, producing additive and sometimes greater-than-additive sedation with a real risk of dangerously slowed breathing, loss of consciousness, and death. Diazepam and its active breakdown products linger in the body for days, so the dangerous window extends well beyond a single dose.
alcohol + alprazolam
criticalAlcohol and alprazolam (Xanax) both depress the central nervous system by enhancing GABA-A receptor activity. Taken together they produce additive — and sometimes synergistic — sedation, slowed breathing, and impaired coordination, which substantially raises the risk of overdose and death even when neither is taken in a large amount.
spironolactone + potassium
criticalSpironolactone makes your body hold on to potassium instead of flushing it out. Adding a potassium supplement, salt substitute, or potassium-loaded diet on top of that can push blood potassium to a dangerous level.
semaglutide + thiamine
criticalThiamine (vitamin B1) is the nutrient the brain exhausts fastest — reserves last only weeks. Semaglutide commonly reduces appetite and can cause prolonged nausea or vomiting, and where intake collapses for long enough, thiamine can run out. Running out causes Wernicke encephalopathy: confusion, unsteady walking and abnormal eye movements. It is reversible if treated within hours and can be permanent if missed. Eighteen cases have been published, fifteen of them following GLP-1 use, and in most the vomiting or poor intake came first.
aged cheese + linezolid
criticalLinezolid is a reversible, non-selective monoamine oxidase (MAO) inhibitor. Eating tyramine-rich foods such as aged cheese while on linezolid can cause a sudden, dangerous rise in blood pressure (hypertensive reaction).
smoking + oral contraceptives
criticalCombining cigarette smoking with combined (estrogen-containing) oral contraceptives raises the risk of heart attack, stroke, and blood clots more than either exposure alone, especially after age 35 and with heavier smoking.

ashwagandha + l-theanine
synergyL-theanine, an amino acid from green tea, produces a relatively quick sense of calm focus by increasing alpha brain-wave activity and gently nudging GABA and other neurotransmitters. Ashwagandha works more slowly, modulating the stress (HPA) axis over weeks of daily use. Because they act through different pathways on different timescales, they are commonly stacked for stress, and there is no known harmful interaction. Importantly, no human trial has tested the combination itself, so the pairing is a mechanistic rationale rather than a proven synergy.
gaba + l-theanine
synergyGABA and L-theanine are often combined in sleep supplements, and the pair may help you fall asleep a little faster and rest more soundly than either alone. The evidence is modest: an animal study and one small uncontrolled human study suggest a benefit, but no controlled human trial has confirmed a true synergy. Both compounds can add to the effects of alcohol and sedatives, so review the combination with your doctor or pharmacist if you take sleep, anxiety, or blood-pressure medication.
caffeine + ashwagandha
synergyCaffeine is a stimulant that raises alertness and cortisol; ashwagandha is an adaptogenic herb that, taken on its own, modestly lowers cortisol and perceived stress in human trials. People combine them hoping ashwagandha will take the edge off caffeine's jitters. That pairing is plausible but has not been tested directly in humans, so the 'calm focus' benefit remains theoretical rather than proven. The combination is generally well tolerated in healthy adults.
caffeine + tyrosine
synergyL-tyrosine is a precursor to the catecholamine neurotransmitters (dopamine, norepinephrine), and caffeine indirectly amplifies catecholamine signaling by blocking adenosine receptors. The pairing is popular as a focus stack, but the direct evidence is limited: tyrosine alone helps preserve cognition under stress or sleep loss, and caffeine aids alertness, yet no human trial has tested caffeine plus tyrosine on their own. The combination is generally well tolerated in healthy adults, with the main cautions involving MAO inhibitors, levodopa, and thyroid medication.
vitamin d + vitamin k2
synergyVitamin D and vitamin K2 act synergistically on calcium metabolism: vitamin D increases calcium absorption while vitamin K2 activates osteocalcin and matrix Gla protein to direct calcium into bone and away from soft tissue. The main caution is for people taking warfarin.
rhodiola + ashwagandha
synergyRhodiola rosea and ashwagandha are both adaptogens that act through different mechanisms. Rhodiola tends to be energizing and anti-fatigue, working on monoamines and the HPA axis, while ashwagandha tends to be calming and helps normalize cortisol. Many people pair them so that rhodiola covers the activating, daytime side of the stress response and ashwagandha covers the calming, evening side. No trial has tested the exact combination, so the rationale is mechanistic rather than proven.
smoking + varenicline
synergyVarenicline (Chantix/Champix) is a partial agonist at the alpha4-beta2 nicotinic acetylcholine receptor — the same receptor nicotine targets. It makes smoking less rewarding, so it is started before your quit date and you may keep smoking briefly at first. Combining varenicline with nicotine replacement (patches, gum) can raise quit rates but increases side effects such as nausea, headache, vomiting, and dizziness.
saffron + curcumin
synergySaffron (Crocus sativus) and curcumin (from turmeric) both have antidepressant effects through partly complementary mechanisms: saffron modulates serotonin and dopamine reuptake and increases BDNF, while curcumin reduces neuroinflammation, supports monoamine balance, and normalizes the HPA axis. A randomized, double-blind, placebo-controlled trial found a saffron plus curcumin combination significantly improved depressive symptoms versus placebo in adults with major depression.
