Semaglutide + thiamine
criticalThiamine (vitamin B1) is the nutrient the brain exhausts fastest — reserves last only weeks. Semaglutide commonly reduces appetite and can cause prolonged nausea or vomiting, and where intake collapses for long enough, thiamine can run out. Running out causes Wernicke encephalopathy: confusion, unsteady walking and abnormal eye movements. It is reversible if treated within hours and can be permanent if missed. Eighteen cases have been published, fifteen of them following GLP-1 use, and in most the vomiting or poor intake came first.
Semaglutide + psyllium
highThese medicines make the stomach empty much more slowly. Indigestible plant fibre that sits in a slow stomach long enough can mat together into a solid ball called a bezoar. In one endoscopy series, four of twenty-three patients taking a GLP-1 medicine had a gastric bezoar of four centimetres or more, and none of the seventy-seven patients not taking one did. A concentrated fibre supplement adds exactly the material these are formed from. Bulk fibre also swells with water and can lodge in the food pipe if taken with too little fluid.
Semaglutide + insulin
highSemaglutide lowers blood sugar, but on its own it rarely causes a dangerous low because it only prompts insulin release when blood sugar is already raised. insulin does not work that way — it drives insulin regardless of the current level. Combined, the glucose-lowering adds up and blood sugar can fall too far, and eating less on Semaglutide removes the buffer that would normally protect against it. Every FDA label in this class carries the same instruction.
Semaglutide + glipizide

Semaglutide + levothyroxine
moderateTaken with oral semaglutide tablets, levothyroxine exposure goes up by about a third rather than down. This is the opposite of the widely repeated claim that these medicines block thyroid hormone absorption. The effect is documented for the swallowed tablet forms of semaglutide, whose formulation changes conditions in the stomach; the injected forms carry no levothyroxine interaction on their labels at all. Over time, more thyroid hormone reaching the blood than intended can push levels higher than the dose was set for.
