Tirzepatide slows how quickly the stomach empties, and that slowing is strongest right after the first dose and after each dose increase. A birth control pill relies on being absorbed promptly and completely; when the stomach holds onto it, peak hormone levels fall by more than half. The FDA label instructs patients on oral hormonal contraceptives to switch to a non-oral method or add a barrier method for four weeks after starting and for four weeks after every dose increase. Contraceptives that are not swallowed are not affected.
